Curriculum

Our Internal Medicine Residency Program offers a multi-layered educational experience that combines clinical care with academic study and teaching opportunities.

  • Elective rotations, including subspecialty clinics and inpatient and outpatient consultations, provide concentrated specialty educational opportunities, with residents working with attendings on a one-to-one basis.
  • All residents attend noon conferences, where a broad range of cases and specialty topics are discussed. All activities are interactive and designed to develop residents’ active learning skills.
  • Journal Club occurs during both the inpatient and outpatient rotations.
  • Patient Safety Conference is held twice per month, attended by a cross-section of departmental faculty representing radiology, pathology, and other medical specialties.
  • Morning Report educational sessions are attended by the senior residents, the night float team, faculty members, and a chief resident.
  • Medical Grand Rounds, with presentations by Albany Medical Center faculty and guest experts on current topics in basic science and clinical medicine, is held twice per month.
  • Attending physicians participate in work rounds, providing a balance of bedside teaching and case-specific discussion.

Call Schedule
PGY-1 residents (interns) on ward teams are responsible for call every fourth night until 9 p.m. Interns work in the medicine units during their night float rotation or in the MICU during their night shifts.

PGY-2 and PGY-3 residents average every fourth night on call until 9 p.m. while on medicine unit rotations.

An admitting team, consisting of one resident and two interns, is scheduled from 6 p.m. to midnight.

There are no evening or weekend call responsibilities during elective or ambulatory clinic rotations.

Ancillary Services
Albany Medical Center and the Albany Stratton VA Medical Center both have extensive ancillary services, including phlebotomists and IV teams, available 24 hours a day, seven days a week. Both hospitals also have Rapid Response Teams that respond to urgent medical situations on the floors to help with expedited care.

Meal Allowance
A meal allowance, redeemable at the hospital cafeteria, is provided to on-call residents at Albany Medical Center. During night float rotations at the Albany Stratton VA Medical Center, meals are provided free of charge in the cafeteria.

Electives
Categorical residents receive an average of 34 total weeks of elective time during their training (approximately eight weeks during PGY-1, ten weeks during PGY-2, and 16 weeks during PGY-3). These include required experiences in Emergency Medicine, Neurology, and Geriatrics. Subspecialty electives are open to all PGY levels, including Rheumatology, Hematology/Oncology, Nephrology, Cardiology, Endocrinology, Pulmonology, Critical Care, Quality Improvement, Women’s Health, Dermatology, Infectious Diseases, and many others.

Floors (Medicine wards)
There are four inpatient teams at Albany Medical Center that are geographically located. Three of the teams see general medicine patients and the third is devoted solely to hematology/oncology patients. Each team includes a resident, two interns, third-year medical students, and a supervising attending.

There are two inpatient teams at Albany Stratton VA Medical Center. These teams consist of one resident, two to three interns, third-year medical students, and a supervising attending.

Night Float
We are fortunate to have a program that is large enough for a night float system at all levels of training. The night float house officer works Sunday through Saturday morning, from 8:30 p.m. to 7 a.m. Night float will be assigned at either Albany Medical Center or the adjacent Albany Stratton VA Medical Center.

PGY-1 residents are assigned up to six weeks of night float and upper-level residents do an average of four weeks per year. The night float team consists of one resident and two to three interns.

Ambulatory Learning
The outpatient experience is designed to prepare residents for excellence in diagnostic and therapeutic practice in General Internal Medicine and specialties. We recognize that at least 80 percent of clinical time for specialty faculty is spent in the ambulatory setting, and that outpatient General Internal Medicine practice is the most important experience in longitudinal management skill development. Faculty preceptors are assigned to each resident in General Internal Medicine clinics, and continuity for your individual panel of patients is stressed throughout the three-year experience.

We utilize a “4+1” system, with one-week clinic blocks every fifth week. This structure avoids interruptions from other clinical experiences, allowing residents to focus on learning and skill development. These ambulatory weeks include both primary care and subspecialty clinic experiences. There is a dedicated ambulatory curriculum focusing on General Internal Medicine topics and evidence-based medicine. Residents are not assigned call duty or weekend clinical responsibilities during clinic weeks.

As a categorical resident you will be assigned a continuity clinic for the entirety of your residency experience at either Albany Medical Center's Internal Medicine Group or the Albany Stratton VA Medical Center.

Scholarly Activity & Experiences
Our Internal Medicine residency is designed to assist all residents in meeting their career goals. Although scholarly activity is required of all residents, research is not the only form of scholarly activity. Depending on your career goals, you will learn to prepare and present for clinical education, develop high-level quality and safety programs, and design both prospective and retrospective investigations as well as actual hypothesis-driven research activities. Residents prepare poster, podium, and written presentations of their work under the mentorship of our faculty. Brief research electives can be made available for senior residents based on the quality of the work proposed after discussion among the resident, program director, and faculty mentor.

Evaluation Processes & Programs
The Internal Medicine Residency Program evaluates each resident for every experience or rotation using a milestone-based evaluation system. PGY-1 residents are evaluated by attending faculty and supervising residents. PGY-2 and PGY-3 residents are evaluated by attending faculty and the PGY-1 residents they supervise. 360-degree evaluations are completed by clinic staff on a semi-annual basis during the continuity clinic experiences. The program also conducts semi-annual, summative evaluations for each resident (performed by the program director or an associate program director). Milestone evaluations are summarized twice per year by the Clinical Competency Committee and results are forwarded to the ACGME.

We place a very high value on information from residents about their experiences in our program. This information facilitates program change. Residents are asked to evaluate attending faculty and clinical rotations after every experience. The results of faculty evaluations allow the program director to choose faculty for the upcoming academic year, in a process of continued improvement. In order to assure the resident staff of anonymity in this process, the residents have designed a Qualtrics evaluation which holds results accessible only to the chief residents, who provide the information to the program on a semi-annual basis.

At the end of each hospitalist team rotation, verbal and written performance feedback is provided to each resident. A Clinical Evaluation Exercise (CEX) is conducted once during the first year of training, and multiple times during the second or third postgraduate year in the continuity clinic setting. An in-training, knowledge-based examination is administered annually for all categorical residents. The results are used as a study guide for residents preparing for ABIM certification and as a curriculum development guide for the program.

Team-Based Learning
A medical knowledge curriculum in residency is traditionally taught in a noon conference lecture format. Lectures are considered passive learning and are gradually being supplanted in many educational settings by active learning methods such as Team-based Learning (TBL) or Problem-based Learning (PBL). These methods foster not just knowledge acquisition, but knowledge application, problem-solving, and teamwork.

Team-Based Learning Lesson Components:

  • Advance reading assignment for each topic
  • Small group work during the lesson including a Readiness Test and Application Exercises
  • Focus on problem-solving, patient cases, and interactive discussion with faculty facilitator

Team-Based Learning Objectives:

  • To impart essential medical knowledge in Internal Medicine and train residents how to apply it to patient care
  • To help residents become excellent general internists
  • To prepare residents to pass the ABIM board-certification exam
  • To help residents further their skills in communication, problem-solving, and teamwork

Team-Based Learning Research at Albany Medical College
We are pioneers in using TBL in graduate medical education (GME). Until 2018 there had been only about 10 studies published on TBL in GME. In February 2018, we published “Feasibility of a Comprehensive Medical Knowledge Curriculum in Internal Medicine Using Team-Based Learning” in the Journal of Graduate Medical Education. It is the first report of such a broad adaptation of TBL in a residency program.

Experience in Quality Improvement & Patient Safety
Aligned with the Institute for Healthcare Improvement Open School, this program helps residents develop skills in systems-based practice, communication, and teamwork. As part of the program, residents, with guidance from a faculty mentor, will learn to:

  • Describe the difference between a Case Review and a Morbidity & Mortality Conference
  • Describe the concept of Case Review Triggers
  • Categorize the cause of a near miss or a suboptimal outcome into either a Process Issue or a Professional Practice Issue
  • Form multidisciplinary teams to improve flaws in processes
  • Enhance their leadership skills
  • Work with non-physician health care professionals
  • Mentor Albany Medical College students
  • Apply principles of “LEAN” to improve quality and reduce risk of harm; become LEAN-certified
  • Describe a problem, its analysis, and its corrective action on a single sheet of paper (A3 process)
  • Observe how Quality Improvement and Patient Safety integrates with:
    • The microsystem of your working environment (e.g. nursing units, clinics)
    • The mesosystem of the Department of Medicine
    • The macrosystem of the Albany Medical Center enterprise and the community we serve
  • Scholarly presentation and publication of their Quality Improvement and Patient Safety Research

Shawn Philip and Ramanathan Veerappan did a QI project that was accepted for presentation at NY ACP in May entitled "ADDRESSING Covid-19 VACCINE HESITANCY AND MISINFORMATION AMONG INCARCERATED PERSONS". They hosted a virtual info session for a local incarcerated population regarding Covid-19 vaccines, that also featured a Q&A with an Allergy/Immunology expert. The incarcerated persons did questionnaires before and after the session, and results were analyzed to see how effective we were at addressing their concerns. They concluded that the incarcerated do value opinions of health care workers and that more and longer sessions are needed to further address vaccine concerns.

Sualeha Khalid presented a poster at NY ACP in May titled 'Maxillary Sinusitis Due to Scedsporium Apiospermum in a Patient with AML Successfully Treated with Surgery and Voriconazole '.

Fatima Mahmood’s poster presented at Y ACP in May was titled, ‘Role of early autoantibodies in the diagnosis of atypical manifestation of Sjögren’s syndrome- A case series”. In this project it was demonstrated how diagnosis of SS was established based on presence of serum anti-SP1, anti-CA6 or anti-PSP antibodies levels. These markers are usually present in the early stage of the disease and can be detected in the blood earlier then anti-SSA levels. These markers can also be used to establish diagnosis in cars when patients do not present with the typical, “dry eyes and mouth”.
