How to Apply

Application Process

• Applications are accepted from October 1 through March 1 of the following year
• Interviews are conducted in March with offers expected no later than April 1
• Program start date is October/November depending on Fellow availability
• Download the PDF application form. Please fill out and submit to [email protected]
• Applications are accepted prior to formal completion of PA Program
• We require your application, CV, interest letter and at least two letters of reference to be eligible for interviews
• Copies of on-the-job and rotation evaluations are highly desired but not required

Admission Requirements

• Completed Application and Personal Essay
• CV and Headshot
• Graduate of an ARC-PA accredited PA Program
  • Master’s Degree Preferred
• Official copy of transcripts from your Undergraduate College/University and PA Program
• Official copy of PANCE Scores sent directly from NCCPA
• Current NCCPA Certification
• Current DEA Licensure
• Current New York State PA Licensure
• Three professional letters of recommendation sent directly from sender’s professional work email.
  • For new graduates, one letter must be from your Program Director
• Family Nurse Practitioners are encouraged to apply with corresponding state licensure, education, and certification requirements.
  • Please note that our providers must be able to care for both adult and pediatric patients in order to be eligible

Applicants are encouraged to apply prior to completion of the above requirements if they are currently eligible for and anticipate completion of all requirements prior to matriculation into the Fellowship. Failure to obtain all requirements voids the offer of Fellowship Training.
