The Albany Med Health System is committed to meeting the medical care needs of your patients. With more than 125 practice locations and 100 medical specialties, our care is easily within reach for your patients.

EpicCare Link

Outpatient referrals to Albany Medical Center practices can be made through EpicCare Link.

EpicCare Link is a secure, easy-to-use web portal that connects community providers to Albany Medical Center’s Epic electronic medical record. Through the Albany Med Health System’s EpicCare Link, you can:

  • Access the real-time updates about your patients
  • Make referrals
  • Request consults
  • Schedule appointments
  • Place orders
  • Send messages

Inpatient Admissions, Transfers, and Consultations

When you call the Access Center to initiate a transfer, please have the following information ready:

• Patient’s name and date of birth
• Patient’s diagnosis
• The specialist required to care for this patient
• Any isolation precautions: contact, respiratory, etc.

If a patient has had any diagnostic studies related to the transfer (i.e. CT, MRI, X-Rays), they will need to come with the patient on a disk with the radiology read.

Emergency Transport

Albany Medical Center's emergency transportation services work with our health care and EMS partners to ensure care is continued en route to our hospital.

LifeNet Air Transport

Upon request, our Access Center can initiate LifeNet, the region's only air transport service. It is available 24/7.

Pediatric Transport

For children not needing air transport, dedicated pediatric ambulances with pediatric nurses and respiratory therapists are available to provide advanced care during transfer to the Children’s Hospital.

Emergency Medicine Community Resources

Albany Medical Center’s Division of EMS works with Albany Medical College's Emergency Medicine program to provide public safety organizations with up-to-date clinical and management education, and expanded-scope pre-hospital medical protocols. Pharmacy

Our pharmacies are located at Albany Medical Center and Glens Falls Hospital.

Albany Medical Center's Pharmacy holds URAC accreditation (expiration date: 2/1/26) as a Specialty Pharmacy and is accredited by the Accreditation Commission for Health Care (ACHC). Our Specialty Pharmacy has dedicated liaisons to help identify and obtain patient financial assistance services and programs, medication adherence programs, personalized refill reminders, on call pharmacist support and home delivery.

Our team works directly with clinical teams to help manage complex, rare and chronic medical conditions that require specialized medication therapy. These conditions include:

  • Hepatitis B
  • Hepatitis C
  • HIV
  • Movement disorders
  • Multiple sclerosis

Albany Medical Center provides free language assistance to patients whose primary language is not English and to those who are visually impaired or deaf or hard of hearing.

The National Institutes of Health has a searchable portal with comprehensive information on medications.

Albany Medical Center employees: We offer reduced prescription co-pays (compared to commercial pharmacies) and 90-day supplies if prescriptions are filled at our pharmacy and you are enrolled in our employer sponsored health insurance plan.
