Albany Med Honored with Quality Award for Providing Outstanding Care
ALBANY, N.Y., May 29, 2014 – Albany Medical Center has been recognized for its commitment to providing high-quality care to Medicare patients by a national monitoring organization focused on improving patient outcomes.
The award from IPRO, the Medicare Quality Improvement Organization in New York State, was given for work done through the Albany Care Transitions Coalition to improve transitions of care for Medicare beneficiaries. Albany Med leads the Coalition and is the only hospital among the 28 Capital Region award recipients.
IPRO noted that the Coalition’s implementation of evidence-based, cross-setting interventions, such as medication management, post-discharge follow up, and patient care plans, resulted in a 30 percent relative improvement in the readmission rates for beneficiaries in the region.
“Improving transitional care from hospital to home, skilled nursing care, or home health care is critical to reducing unnecessary hospital re-admissions of patients – allowing them to heal and recover more quickly in the most appropriate setting,” said Steven M. Frisch, M.D., executive vice president and hospital general director.
Citing the Coalition’s work, Clare B. Bradley MD, MPH, Senior Vice President and Chief Medical Officer, IPRO, said, “These healthcare providers have demonstrated that, through their willingness to collaborate and make quality improvement an organizational priority, they can reduce re-hospitalizations and help ensure that patients experience the best transitions of care, across all settings.”
The 2014 IPRO Quality Award builds on other quality awards given to Albany Medical Center recently. Earlier this month, Albany Medical Center was one of two New York State hospitals recognized recently by the American Heart Association/American Stroke Association for its success in providing quality resuscitation care for both adult and pediatric patients who suffer cardiac arrests in the hospital. In April, the Albany Med Faculty Physicians Group was recognized by the National Committee for Quality Assurance for providing high-quality, coordinated care to patients using evidence-based, patient-centered processes.
