Access Made Easy

Albany Medical Center is committed to meeting the health care needs of the patients and families we serve across our 25-county region—24 hours a day, seven days a week, 365 days a year, no exception. This includes ensuring that we are able to respond immediately, at any hour, to your needs and those of your patients and, when necessary, coordinating a smooth and safe transfer of service.

To transfer a patient, including high-risk mothers and babies to Albany Med,
call (518) 262-4660.

Upon calling the Access Center to initiate a transfer for your patient, our Access Coordinators will identify who you are and where you are calling from.

Please have the following information ready:

• Patient’s name and date of birth
• Patient’s diagnosis
• The specialist required to care for this patient
• Any isolation precautions: contact, respiratory, etc.

If the patient has had any diagnostic studies related to the transfer (i.e. CT, MRI, X-Rays), they will need to come with the patient on a disk with the radiology read.

All calls to the Access Center are recorded for quality management.
