A 64-year-old caucasian man was admitted with a non-productive cough and right hand and forearm swelling. He admitted to 25-30 lbs weight loss during the past 6 months. He also had occasional ataxia, slurred speech and blurry vision. He denied fever, chills, chest pain, shortness of breath or night sweats. He was a known smoker who had smoked 2-3 packs per day for 45 years.

What is the most likely diagnosis considering the following roentgens? What has occurred in the progression of the two films? What would you consider in his prognosis?
