A 42-year-old Hispanic male inmate with dyspnea

A 42-year-old Hispanic male inmate presented with a 3 month history of 30 pound weight loss and fatigue, and a three week history of increasing shortness of breath and a nonproductive cough. On admission, his vitals were temperature of 98.2 degrees Fahrenheit, pulse 77, blood pressure of 120/80, respiratory rate of 40, oxygen saturation of 96% on room air and appeared in mild distress. Exam revealed a well-developed man (wt. 177 pounds) appearing stated age with moderate facial, hands and feet wasting. Chest exam showed decreased breath sounds at the right base, and mild scattered expiratory wheezes. His past medical history included HIV, diagnosed in 1985, most recent CD4 count of 911, on anti-retroviral therapy, Hepatitis B and C, collapsed right lung with bilateral empyemas in 1980, hyperlipidemia and gastroesophageal reflux disease.
