A 64-year-old woman with congestive heart failure
due to dilated cardiomyopathy

The patient is a 64-year-old caucasian woman with a history of congestive heart failure secondary to dilated cardiomyopathy, who also has hypertension, mitral valve regurgitation, aortic valve regurgitation, and history of coronary artery bypass grafting.
