Dear Graduate Student Applicant,
Thank you for your interest in graduate studies in the Center for Cardiovascular Sciences (CCS) at Albany Medical College. The following links include information about the CCS graduate program and applications forms that can be submitted either by e-mail or Fax:
eApplication - A fillable application that can be completed in MIcrosoft Word and returned to us at hobbw@mail.amc.edu. Please follow the directions on the final page of the domestic application for submission of transcripts, GRE scores and recommendation forms. If you are not a U.S. citizen, you must also complete the International Applicant Supplemental application.
AMC Rec - A recommendation form that should be e-mailed to each of the references listed in your application.
Print PDF - If you don't have access to Microsoft Word, this PDF of all application forms can be printed, filled in by hand and faxed to us at 518-262-8101.
