We understand that appointments are missed due to unforeseen circumstances. Whenever an appointment needs to be canceled, please contact us 24 hours in advance so that we may reschedule another patient in that time slot. If you are more than 30 minutes late for your appointment, we reserve the right to reschedule your visit for another date.
Referrals
It is the patient's responsibility to obtain a current referral which is needed at the time of the visit. We will work with you to obtain a current referral. However, if your referral is not current at the time of your visit, you will be required to sign a waiver and you will be responsible for the bill.
