Upon referral, patients are evaluated by a multi-disciplinary team of cardiologists, cardiothoracic surgeons, critical care specialists and anesthesiologists.  The team consults with practitioners who have extensive experience in nursing disciplines, heart failure and heart transplant coordination, nutrition, social work, pharmacology, immunology, infectious diseases, psychiatry and clinical pathology.

Albany Med’s staff is available 24-hours a day to help patients to help themselves through proven practices of chronic disease management.
