Medical Social Work- Case Management
Inpatient Medical Social Workers assist patients while in the hospital with a variety of issues including entitlements, housing issues, referrals for homecare, and end of life issues. This hospital-based staff is an integral part of the health care team, often providing intensive services to patients while they are in crisis or at medically difficult times. The inpatient and outpatient case managers work collaboratively to provide a continuity of care when patients are admitted and/ or discharged.
