General Case Management ServicesCase Management is an organized system that assists patients in accessing services that they need. By utilizing a team approach, case managers involve the patient, their supports, health care providers, and other agencies in a process that targets the patients specific areas of concern.
Case Managers meet with patients to address their concerns by assessing medical, psychosocial and physical needs. Then, case managers will work with patients to develop a service plan that addresses the needs of the patient and his/her family. As needs change, case managers provide patients with continued support, education and advocacy at a level of care that best suits the patients needs. Staff is available to assist with an array of needs including: family and domestic issues (ie: disclosure, partner notification, domestic violence, permanency planning), health insurance, home care, housing, food, income assistance, SSI and disability, employment, and referrals for mental health and substance abuse services (both within the program and externally).
General Case Management provides intake and assessment services as well as referrals to other Albany Medical Center and community programs. If a need arises during an appointment with a medical provider, patients can meet with our Triage Case Manager. For those patients who receive case management services at community agencies, our Liaison Case Manager is available to facilitate communication among all of the patients service providers, to assist the patient in coordinating his/her care.
Family Case Management provides more intensive services for those patients and their families who have complicated needs and require long-term, on-going support from a case manager. In conjunction with the patient, Family Case Managers assess and address the psychosocial needs the entire family unit.
