Making Preparations
Staff members at Albany Medical Center Hospital work together to make your transition from hospital to home as smooth as possible. They often begin planning your discharge when you are admitted. While this might seem strange to you, it is an effective way to prepare for any care you will need after leaving the Hospital.
Discharge Planning
After evaluating a patient's needs and consulting the patient's physician, the nurse, with the assistance of the Case Manager or Social Worker, will help determine what (if any) services must be provided once the patient goes home. This might include physical therapy, nutritional guidance, or special medical equipment. In addition to arranging these services, you will also be provided information on insurance coverage. To request additional information or schedule an appointment with a Case Manager or Social Worker, call the Case Management/Social Work Department at (518) 262-3012 during regular business hours.
Your Discharge
Your physician will tell you when you are ready to leave the Hospital. To avoid any last minute confusion, try to arrange a ride home the day before your discharge. The person picking you up should try to arrive before 10:00 am. They can park in the AMC parking garage and use one of your vouchers to receive free parking.
Private Duty Nursing
Information on Agencies offering private duty nursing services is available in the Nursing Scheduling office at (518) 262-3487. Although Albany Medical Center is providing you with information on area agencies offering private duty nursing services, arrangements to employ private duty nurses are completed directly by you or your family?not Albany Medical Center. Albany Medical Center is not responsible for private duty nursing fees. You or your family must handle any related charges. Patient Financial Counselors can help you determine whether your insurance or HMO plan will cover the expense of a private duty nurse. You can contact them at (518) 262-3645.
