Albany Med Nationally Recognized for Providing Patient-Centered Care
ALBANY, N.Y., April 24, 2014 – The Albany Med Faculty Physicians Group has been recognized by a national certifying group for providing high-quality, coordinated care to patients using evidence-based, patient-centered processes.
Four practices within the Albany Med Faculty Physicians Group -- HIV Medicine, General Pediatrics, Internal Medicine/Pediatrics, and the Internal Medicine Group -- received Level III certification (the highest level possible) by the National Committee for Quality Assurance’s Patient-Centered Medical Home Program for embodying all the characteristics of the medical home model. These include promoting care coordination and communication among practitioners, patients and their families to ensure that decisions respect the patients’ wants, needs, and preferences and ensuring patients have the support they need to make decisions and participate in their own care.
Albany Med’s Faculty Physicians Group, with more than 400 experienced physicians, is the largest physician group in the region and offers the greatest number of medical specialty services.
“This recognition affirms Albany Med’s commitment to putting the patient first while providing high-quality, coordinated care delivered by teams of exceptional providers,” said Ferdinand J. Venditti, Jr., M.D., vice dean for clinical affairs and president of the Albany Med Faculty Physicians Group.
To receive this national recognition, which is valid for three years, the Albany Med practices demonstrated the ability to meet the program’s key elements, embodying characteristics of the medical home. Additionally, Albany Med’s standards align with the joint principles of the patient-centered medical home established with the American College of Physicians, the American Academy of Family Physicians, the American Academy of Pediatrics, and the American Osteopathic Association.
